You also receive a laminated business type card, and a printout of sticky labels of your 2-dimensional bar code for other documents.
Advantages: With oversight provided by your currently active proxy/agent, only your most recently revised forms and videos will be presented to your physician, emergency medical personnel, and others. If you want to add or change anything, you need only do so on what is uploaded. There is no need to contact every possible doctor, hospital, proxy/agent, and loved one.
