A Basic Membership in Caring Advocates includes registration of all your Advance Care Planning documents, which includes the forms, Advance Directive to Permit Natural Dying and Physician's Orders to Permit Natural Dying. The cost is $20 per year, including the first year.
IMPORTANT NOTE: You must be a member of Caring Advocates to request Caring Advocates to send the forms for Natural Dying to your physician. Your form will have your Unique Identifying Number (UIN). Forms can also be obtained by consulting a Caring Advocates--Certified Planning Professional. The reason we do not distribute useable forms to the public is because we insist on professional oversight for their use. However, you can download evaluation copies of these two forms for Natural Dying by clicking on the image below:

A Regular Membership in Caring Advocates includes in addition:
- The book, Peaceful Transitions: A Successful, Compassionate Strategy for Life's Final Challenge (a $25 value);
- One deck of My Way Cards and, after you complete sorting them and send us your decisions, a My Way Cards-Living Will that is ready for you to sign and have witnessed (a $30 value);
- Your state's Proxy Directive / Power of Attorney for Health Care;
- When you request it, we will send you a Thirst Reducing Aid Kit (a TRAK);
- Access to consult our professional staff for advice and support.
The Regular Membership costs $100, a portion of which may be considered a tax-deductible donation by the person who prepares your taxes.
Caring Advocates will try to work with everyone, regardless of the ability to pay.

$20 | Medallion + Registration (if purchased). The cost is $20.00 a year, including the first year. Your medallion will be engraved with a toll-free number and a web address for internet access so that authorized people can request immediate retrieval of the latest version of all your documents stored in a national registry. That way, the document will be available when needed. Also, there is no need for you to send copies to all your physicians, proxies, and loved ones every time you modify or change a document; you can just let them know (electronically) that you made some changes. Importantly, these people can also be notified electronically, in case of an emergency. If you at some point require a Thirst Reducing Aid Kit, we will provide one at our cost.

Determined on an individual basis | This is our finest service. It is particularly appropriate for patients with early dementia. All of the above plus two or more hours of interviewing that can be recorded on audio or video-as a Planning Professional A) assesses your ability to make medical decisions, and B) documents your end-of-life wishes. Other strategic documents can be added, if required. |
